We believe that quality outcomes are a team effort and our staff works proactively with neonatal providers to attain the best possible outcome for each child.

One of the biggest challenges in neonatal practice today is the wide variation in practice patterns and patient outcomes. We partner with providers to facilitate and coordinate health care services to decrease the unnecessary gaps and fragmentation in care that can occur. Our staff routinely shares information that supports an evidence-based approach to treatment and our neonatologists welcome the opportunity to discuss this information with their neonatal colleagues.

 
 

ProgenyHealth provides the following Neonatal Care Coordination services to our clients:

  • Inpatient utilization review services speaking directly with the clinical staff at the hospital on a regular basis to obtain the current clinical status and medical plan of care on each infant. ProgenyHealth’s medical management program has been awarded full accreditation in Health Utilization Management by URAC.
  • Peer to peer calls to the attending physician as needed by a ProgenyHealth physician advisor to discuss an infant’s clinical status and expected length of stay to promote fair and efficient utilization of resources and an evidence-based approach in daily clinical practice.
  • Distributing clinical practice guidelines based on current peer reviewed literature and expert panel consensus to decrease variations in care. These guidelines also serve as an additional resource to neonatologists to stimulate discussion with their colleagues and to promote thoughtful consideration for incorporating them into their current NICU practices.
  • Engaging families during their infant’s inpatient stay to become active participants in the health care decision making process through education and empowerment provided by ProgenyHealth case managers
  • Reviewing all requests for injectibles and pharmaceutical products to make sure they meet standard prescribing guidelines for medical necessity
  • Early implementation of discharge planning services to ensure that all necessary DME and health care services are in place prior to an infant’s anticipated discharge date.
 
 

We believe at ProgenyHealth that care management should not end when a premature or medically complex newborn is discharged home from the hospital. A major reason medical costs continue to rise throughout the first year of life is due to a family’s inability to navigate our complex health care system.

We differ from other programs in the marketplace in that we continue to work with families after discharge from the hospital throughout the first year of life.

Families have a dedicated case manager to assist them during the inpatient stay and after discharge from the hospital to meet the individual health care needs of their infant. The goal of the case manager is to put interventions in place which will result in an improvement in the process, content and effectiveness of the health care services received by their infant.

All families referred for case management services receive a call from their case manager introducing ProgenyHealth services. The case manager performs a needs assessment to determine what level of intervention is most appropriate for the family and assembles and mails a customized educational packet based on the infants particular clinical situation.

For infants with more complex needs, an individualized plan of care is developed in conjunction with their primary care physician and these customized care plans are available on the web for families and their physicians to access at a time that is convenient for them.

Our secure web-based application provides our clients with real-time access to their data 24/7 to meet their reporting requirements. Reports can be customized to meet the individual needs of our clients.
