Supporting women, infants, and families

Maternity Case Management

At-risk mothers-to-be are paired with a dedicated case manager to help resolve risk, monitor prognosis, and ensure all available resources are available for the healthiest pregnancy possible.

NICU Utilization & Case Management

If a NICU admission occurs, our NICU Utilization Management team collaborates with facility physicians and clinicians to standardize care, and ensure appropriate lengths of stay and levels of care. NICU case managers connect with moms and infants to support discharge planning, screen for postpartum depression, and provide education and emotional support during the NICU stay.

Intelligent Platform

When there is a NICU stay, Baby Trax®, our predictive and prescriptive analytics platform, ensures the application of accepted guidelines to drive better outcomes at a lower overall cost of care.

Postpartum

Our whole-person approach provides post-pregnancy case management, including scheduling doctor visits, monitoring for postpartum depression, hypertension and other complications, and preparing women and their families for return to work.

Non-clinical Gaps in Care

Our team identifies proactive solutions for SDoH issues and other non-clinical gaps in care and works with moms to readily access the care, benefits, and community resources they need to thrive.

Mobile App

Our easy-to-use mobile app empowers patients to self-enroll, start a personal risk assessment, create custom birth plans, and access curriculums. They can track their pregnancy progress and health appointments, and connect with their case manager.
